Baptism/Bautizo

GUIDELINES AND REQUIREMENTS FOR THE SACRAMENT OF BAPTISM

For Parents 
• Submit a Baptism Registration Form and Birth Certificate
- Baptism Registration forms can be picked up at the rectory or printed from our website. Forms can be mailed in, emailed at 
raguilar@archchicago.org or dropped off at the rectory during normal business hours.
• Attend a Baptismal Prep Session – check with the rectory or website for available session dates. 
Generally prep sessions are held in English once a month on the first Tuesday at 6:00 pm at St. Dismas
Church and in Spanish once a month on the second Tuesday at 6:00 pm at St. Anastasia Church.
• Stipend of $75 due at the time the registration form is submitted.
• Baptism dates are pre-scheduled throughout the year, generally once a month on a Saturday at 12:00 pm. 
Baptism dates are confirmed after the Baptismal Prep Session has been attended and all other 
requirements are met.
• Parishioners actively participating in Sunday Mass.
• If you are currently not a member at any parish, we invite you to register. Registration forms are available 
on our website, church vestibule turnstiles, and at the rectory.

For Godparents
• Attend a Baptismal Prep Session whether at LFP or their own parish. If class is taken at another 
parish, must provide a letter from their parish confirming the GP is eligible to baptize and has permission 
to baptize at LFP. If class is taken at LFP, provide a copy of GP’s confirmation certificate.
• Be at least 18 years of age.
• Have received the sacraments of Baptism, Holy Eucharist, and Confirmation.
• Permission from their parish to baptize at LFP (if from another parish) if not a parishioner of LFP.
• Be a member of a Catholic Parish “in good standing” (attending Mass weekly).
• Godparents, if they are a couple, must be married in the Catholic Church.
